St. Anthony's Supportive Housing
Sirona Recovery’s housing programming provides strengths-based case management to empower members of our community on their path toward building and sustaining a more meaningful life.

St. Anthony’s Place offers permanent supportive housing with 60 units managed by Heartland Housing. As part of the Housing First initiative, this housing is offered to individuals with special needs who are experiencing chronic homelessness through a low-barrier approach without preconditions of treatment or sobriety.

 

Sirona Recovery provides supportive services through onsite Case Managers who assist in the areas of mental and physical health, substance abuse, education, and employment. Several of the units in the building are MyHome project-based subsidies through the Milwaukee County Housing Division. Other units can be occupied by residents with Section 8 vouchers, MyHome tenant-based vouchers, and fair market.

  • Individualized service plans are created with residents using a person-centered approach that emphasizes housing stability.
  • One-on-one and group services are offered to residents including building skills for daily living, social skills, conflict resolution, and general wellness.
  • Referrals to community providers are offered including substance use and mental health treatment, employment, medical, financial, education, food, clothing, peer support, and leisure activities.
  • Case Managers assist residents with paperwork and conduct home visits needed to maintain their housing and serve as the main point of contact with the landlord to help address any issues.
  • Staff coordinate with other case management providers to ensure a comprehensive approach to helping the resident work toward goals and maintain housing.
