CCS Care Coordination is a voluntary recovery-focused and client directed program of Milwaukee County’s Behavioral Health Division (BHD), Community Access to Recovery Services (CARS) network.
CCS is a recovery-focused Medicaid program for adults with mental health and/or substance use diagnoses. Sirona’s CCS program is open to Milwaukee County residents, age 18 and older confronting substance abuse, mental health, or co-occurring substance abuse and mental health conditions. This voluntary and client-directed program utilizes Care Coordinators to help clients develop a recovery plan that identifies the services that will help them to achieve their recovery goals and to assist in accessing these services. Milwaukee County residents covered by Medicaid are eligible to participate in CCS programming.
